Talk with a registered dietitian about nutrition and how to make healthy eating and physical activity part of your life.

If you’re living with Type 2 diabetes, you may need to change some things about your daily routine to manage your diabetes.

Emotional health is important too. Talk with a health professional for extra support.

People with diabetes are two to three times more likely to have depression than people without diabetes.2 When left untreated your diabetes and mental health can worsen.

Work with your doctor to see what medications you may need.

Managing your diabetes may include medications and learning how to administer them. Talk to your doctor and review your health plan’s preferred drug list.

You can also get convenient home delivery on a range of diabetic products including insulin pumps and supplies.2 Ask your doctor to fax your prescription to Southwest Medical Pharmacy and Home Medical Equipment to receive supplies through this program. Extra help is available for members who have diabetes or other complex health conditions.

Our Care for Me Program nurse serves as a single point-of-contact after a hospitalization, emergency room or urgent care visit. The nurse will work with you, your primary care provider (PCP), caregiver and other health care professionals to help manage your health care needs.

Some of the program services include:

  • Access to a registered nurse
  • Care coordination
  • Self-care advice and management
  • Medication review
